There are two modes: Classic and Story.

Classic mode is more straight forward: click all the bad pumps.

Story mode requires more diligence. Based on the historic work of John Snow during the 1853 Cholera outbreak in London, investigate each block to find the Cholera Contagion! 'Take Care' of the citizens and you'll be rewarded REPUTATION. Use your REPUTATION to build CREDIBILITY at the HOSPITAL--- among other bonuses. With enough CREDIBILITY, you can convince the CITY GUARD to remove the flagged pumps, or you can employ the help of HENRY WHITEHEAD to also help you. But be prepared to answer questions about Cholera and Victorian-era anesthesia, your speciality. But investigate carefully, as any number of pumps can be poisoned, and the populace can only help so much.

`A consideration of the pathology of cholera is capable of indicating to us the manner in which the disease is communicated. If it were ushered in by fever, or any other general constitutional disorder, then we should be furnished with no clue to the way in which the morbid poison enters the system; whether, for instance, by the alimentary canal, by the lungs, or in some other manner, but should be left to determine this point by circumstances unconnected with the pathology of the disease.  But from all that I have been able to learn of cholera, both from my own observations and the descriptions of others, I conclude that cholera invariably commences with the affection of the alimentary canal. `  -John Snow ( http://www.ph.ucla.edu/epi/snow/snowbook.html )

I was having some trouble getting the Audit system to work, no matter how much I took care of people there was no way to know what my reputation was with the district. But I really like this idea and would love to see more of it.
